A local tattooist has obtained an Infra-red Coagulator and is advertising"
virtually scar free tattoo removal"
My understanding is that this equipment is not selectively damaging and
certainly does not operate within the thermal relaxation time of tattoo pigment
and is therefore not able to achieve selective photothermolysis.  As such there
should be a significant incidence of scarring.

This equipment has been marketed to dermatologists in the U.S.

Has anyone used or  seen complications with this equipment.

One of the residents at UCLA/Drew (A. Paul Kelly is chairman)
presented a paper at the national medical association conference about two or
three years ago. Her experience with this instrument which was used in
several black and hispanic patients was not good and, as I recall, left
scarring. You might try to contact Dr. Kelley. I don't believe that paper was
published.

I have used the infrared coagulator and can assure you that it does produce
scar.  Basically, this device produces thermal tissue damage resulting in a
wound similar to a cryosurgery or curretage scar.  The advantage of the
device relates to the ability to confine the tissue necrosis to the diameter
of the saphire tip used (usually 6 mm).  Depth of injury is controlled with
pulse width (0.75 seconds = 0.25mm tissue necrosis; 1.25 sec = 1.16 mm tissue
necrosis). Some studies have compared the infrared coagulator to the CO2 laser
- basically, the laser performed better because of improved visualization and
hence more complete tattoo removal; however, the IRC can be desirable because
of the lower cost, ease of use, bloodless field, and no smoke plume.  Scarring
between the two modalities was basically equivalent.

I agree with Dr. Starr. Have had one for 4-5 years and it does scar.Now I use
it mostly for digital mucinous cysts----great results.

I have seen the results of an infra-red coagulator used by another 
physician for the treatment of aks and bccs.  The scar results are 
significant.  I am sure they would be the same when used for tattoos.
